“Patient input and feedback is required as patient and physician work as a team with the ultimate goal being the return to health”
Lifestyle and diet are two of the major impediments to good health, therefore, counseling and lifestyle modification are essential in our modern day life. Patients are therefore advised to make healthy lifestyle choices as physical, emotional, environmental and nutritional factors all affect health.
Prior to being treated, patients are required to fill out questionnaires detailing the history of their health, priorities, and present concerns. This information is evaluated prior to any appointment so that an effective plan of treatment can be drawn up, prioritized with the patient, and implemented. Treatment is then monitored in scheduled follow-ups, to determine the progress being made. Changes in the treatment are made as required to insure that the patient moves towards the restoration of their health.
Patient input and feedback is required as patient and physician work as a team with the ultimate goal being the return to health. Patients are also made aware of their personal responsibility to make the changes necessary for this restoration to occur.
Each patient is treated on a personal and individual basis as the history and health of each is unique.
All patient information is totally confidential in accordance with the current confidentiality laws of the Province of Ontario.
